Wheels Class Pass Request
Class Pass Request Form | Fun Trips

Please fill out this form two weeks prior to your trip. You may then hit the Submit Request button and the information will be sent electronically to Wheels. This will allow us time to plan a personalized trip and to mail you a copy of the Class Pass.  If you're having a problem sending this form, please contact us at info@lavta.org and explain the issue.

Wheels will plan the best route for your group using regular fixed route bus stops.  Please remember the following:

  • Trips must be taken between 9 AM and 2 PM.
  • Class Pass is valid only on Wheels fixed route buses.
  • Class Pass is valid only for the date issued.
  • Groups greater then 30 need to be split into 2 or more smaller groups.
